Acute Episode: Deliberate Transitions “Hospital to Home”
Programs A & B
One hour face to face (home or facility) plus two hours telephonic & online. Patient and caregiver complete HA2T & PAM.
- ++24/7 telephonic/online consults outside office hours: $80/hr, prorated in 15 min
- ++Additional home or facility visits: $100/hr (1 hr minimum) in Clallam, Jefferson or Pierce counties, plus $40/hr commute

Individuals & facilities
For individuals in service II, and for community hospitals, skilled nursing facilities and adult family homes by contractual agreement. HIPAA-compliant redacted PHI transmissions via Citrix ShareFile.
