The 1 GHC Binder · Greater Health Community at MyTown, USA

The Pragmatic Health& Life Paradigm.

MyGHCP is a Health & Life Plan eBook. As you complete evidence-based quick self-assessments, it graphically shows you the barriers to your optimal health and the next best steps to maintain or readjust. It works best when you are transparent and inclusive with your family, friends and confidants.

01 — Why patients need a GHCP

When an industry's safety culture is broken, the burden falls on you.

Healthcare harm and death is now the third leading cause of death in the United States, and it isn't regulated and investigated like the aircraft and automobile industries are.

The Patient Safety Act of 2005 and the Affordable Care Act of 2010 don't require full investigations and accountability for unexpected harm. So patients and families must identify the hazards themselves: “Do it yourself” and “Demand Better”.

The mission of TriUnity Health Institute is to help individuals and families accomplish the TriUnity: patient safety, coordinated care and positive outcomes.

Institutional health co-ordination

Autonomy
The silo phenomenon, least co-operation
Integration
Most co-operation. Few US systems have this

Your task: decide how vertically and horizontally integrated each of your providers is. The further right they are, the less work you have to do in your GHCP to get coordinated care.

02 — The GHCP Self-Assessment

Assessing is the easy part. Fixing the deficits is the work.

Step 1 · Subjective self-assessment

A quick self-rating of twelve categories of individual health management: the Healthogram on page one of the 1 GHC Binder.

Step 2 · Objective self-assessment

The same twelve categories, measured with established evidence-based rapid health assessment tools on page two.

A · The HA2T report

Twelve categories of internal & external characteristics

  1. 01Finances
  2. 02Cognitive
  3. 03Physical ability for Activities of Daily Living
  4. 04Depression or other mental health dysfunction
  5. 05Neurological: dementia or other organic brain dysfunction
  6. 06Insurance

B · Global Determinants I–IX

Your global determinants trump your HA2T.

Seven or nine dyadic relationships in the Primary Triad: patient A, family & friends B, and experts & providers C, plus each one's relationship with Home (VII–IX). With two of three core dyads positive, you can technically still reach optimal health.

Quick Instructions · rating key

?

See provider

Discuss with your provider, consultant, or a family counselor or therapist.

03 — After all is said and done

“For every uncertainty “0” or distancing “--”, and every instability or cutoff “=”, there is a potential deactivation to one's goal of optimal health.”

If your Global Determinants I–IX are essentially all positive, you should have optimal health. With a serious or life-shortening condition, it is your key relationships with family, friends, providers and consultants that matter most.

Every deactivation or cutoff is an “incompleteness or triangle” waiting for the missing idea. Those are your starting points. The GHCP's strategy tools are the “Working the Triangles Binder and Template” and the CGBAO.

04 — Four ways to use the GHCP

In order of increasing urgency.

Life-long learning log

Your personal, family-oriented electronic record and guidance system. Keeping it prepares you for everything below and finds illness earlier.

A new illness, injury or disability

A serious curable illness, a temporary or permanent disability, or a new chronic condition that needs new knowledge, supports and self-management skills.

Caring for someone else

A family member or friend can't manage their condition, and you become their delegated GHCP manager / caregiver (see MYDECAF).

Hospital to home

You or a family member is hospitalized, or facing a new life-threatening diagnosis: ensure the TriUnity through “Triunity Deliberate Transitions”.

05 — Emotional Fusion & the Rescue Triangle

From dyadic polarisation to triadic thinking.

Dr. Murray Bowen's Family Systems Theory underscores emotional fusion and differentiation. Emotional fusion is a state where people become enmeshed with the emotions and needs of others, blurring boundaries and losing individual autonomy.

In the Rescue Triangle, danger on one side makes thinking, objectivity and facts disappear. The way out is a healthy triadic process of thinking, feeling and doing: sustaining rational thinking while lowering emotional overload, so there is less stress and chaos at home.

When your go-to providers are unavailable and you can't get timely appointments, you are in the Rescue Triangle, and your disease control ends up in the ER.
